ABOUT THIS ROLE
<p>The Registered Nurse - Care Coordination and Integrated Case Management (CCICM) utilizes an interdisciplinary approach to coordinate care required by patients with chronic, catastrophic, or complex high-risk or high-cost health issues. They provide structure and standards to support interprofessional collaboration and optimal utilization of healthcare resources.</p><h3>Major Duties</h3><p>The Registered Nurse - CCICM assures the physical and mental health, and psychosocial needs are addressed to enhance Veteran's wellness, level of functioning and quality of life with the goal of providing safe, efficient, and cost-effective interventions integrated throughout the continuum of healthcare. Major Duties include, but are not limited to: Utilize a framework that provides standardized definitions and tools designed to integrate care horizontally and vertically to include but not limited to: bridging medical care and mental health care; transitioning inpatient care and outpatient care; and integrating community care and VA care thereby creating a seamless continuum experience for Veterans. Provide support at the unit, service, and organizational levels. Serve as a representative on medical center and service committees. Manage the daily operations of the program and assists with ensuring facility-wide compliance with VHA Directive 1110.04, Integrated Case Management Standards of Practice. Assist with the development of policies and procedures, contributes to the community of practice across the Veterans Health Administration (VHA) through use of a data-driven quality improvement approach to case management practice with established goals and outcomes to evaluate and document case management effectiveness. Identify and monitor case management metrics in collaboration with health informatics and prepares program and performance metric reports for VA medical facility leadership on a quarterly basis. Assist with the completion of facility readiness assessments and other assignments as tasked locally or through VISN actions. Oversee the Care Coordination Review Team (CCRT), an inter-professional and inter-departmental team comprised of specialty case management staff including primary care and mental health staff with experience in care coordination that conducts high-level reviews of cases needing special attention. All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service. Various; that may include: both 8 and 12 hour shifts, days, nights, weekends, and holidays. Schedule will ultimately be determined by Supervisor and/or needs of the Facility. Telework: Not available. Permanent Change of Station (PCS): Not authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
