ABOUT THIS ROLE
<p>The Registered Nurse - Patient Aligned Care Team (PACT) Care Manager delivers and improves holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned.</p><h3>Major Duties</h3><p>The Registered Nurse (RN) - Patient Aligned Care Team (PACT) Care Manager works in partnership with an assigned panel of patients and team members for effective disease and chronic care management. They use the nursing process and evidence-based practice to collaborate with the expanded team, internal and community-based services, in developing the patient driven care plan for prevention and management of diseases. Major Duties include, but are not limited to: Delivers efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. Utilizes core elements of performance including knowledge and active participation in unit/service level quality improvement processes, such as the customer service program. Promotes the medical center mission, vision and values; compiles the Equal Employment Opportunity programs, partnerships, customer service standards and Veterans Affairs policies and procedures. Works in partnership with an assigned panel of patients and team members for effective disease and chronic care management. Responsible for collection of data and implementing evidence based changes to practice based on data results. Uses the nursing process and evidence-based practice to collaborate with the team (Patient, Primary Care Provider, PACT RN Care Manager, Health Technician, Pharmacists, and Clerical Associate) and expanded team (family member or caregiver, consulting providers, internal and community-based services involved in the care of the patient) in developing the patient-driven care plan for prevention and management of diseases. Collaborates with the services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs. Provides evidence-based patient and family health education with a focus on self-management, prevention, and wellness. Initiates follow-ups as recommended while administering the prescribed therapies using appropriate judgement. All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service.
