ABOUT THIS ROLE
<p>Heroes' Corner is an 18 bed Palliative Care/Hospice Unit which is a part of the Community Living Center. It provides care to Veterans who are receiving palliative chemotherapy, palliative radiation, palliative rehabilitation, and Veterans who are at end of life. Veterans may discharge home, to community facilities, or stay on the unit for end of life. Heroes' Corner provides symptom management, wound care, IV therapy, and more.</p><h3>Major Duties</h3><p>Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assume responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Collaborate with the Minimum Data Set (MDS) coordinator to ensure documentation is accurate and reflective of the Resource Utilization Group (RUG) score. This position potentially requires flexibility in schedule and assignments.
