ABOUT THIS ROLE
<p>The VA Sierra Nevada Health Care System (VASNHCS), within VISN 21, is seeking a highly skilled Mental Health / Inpatient Clinical Pharmacy Specialist (CPS) to serve as an advanced practice provider delivering evidence-based comprehensive medication management (CMM) to Veterans in both inpatient mental health and acute care settings. The CPS functions with prescribing privileges under an approved scope of practice and provides direct, indirect, and interdisciplinary clinical care.</p><h3>Major Duties</h3><p>Major Duties Clinical - Direct Patient Care - Functions as an advanced practice provider delivering CMM aligned with national treatment guidelines, formulary requirements, and local policies. - Prescribes, monitors, and adjusts psychiatric and inpatient medications, including complex mental health regimens. - Orders, interprets, and follows up on labs including toxicology screens; implements necessary therapy changes. - Performs mental health assessments and screenings (e.g., depression, PTSD, substance use, suicide risk). - Provides education to patients and caregivers regarding medications and devices. - Performs medication reconciliation at admission, transfer, and discharge. - Attends daily inpatient team rounds; completes patient profile reviews. - Manages acute and chronic disease states common in inpatient settings, including pain, anticoagulation, infectious disease collaboration, PK/renal dosing, TPN, DKA, CHF, COPD, diabetes, and more. - Evaluates need for DVT prophylaxis, immunizations, follow-up care, and transitions between care levels. - Conducts physical assessments and refers patients appropriately based on acuity and treatment needs. Clinical - Indirect Patient Care - Reviews medication regimens for safety, appropriateness, interactions, contraindications, and therapeutic outcomes. - Monitors and documents adverse drug reactions, allergies, and compliance issues. - Provides drug information consultations, e-consults, and literature reviews for interdisciplinary teams. - Reviews Pyxis overrides, performs IV-to-PO conversions, and assesses patients on high-risk medications (e.g. lithium, valproate, DOACs, insulin, vancomycin). - Participates in quality improvement, RCA/HFMEA, inpatient order set development, and policy support. - Maintains BLS and ACLS certification; upholds knowledge for thrombolytic administration in stroke emergencies.
