VISN 20 Clinical Resource Hub (CRH) Nurse Practitioner (Tele-Primary Care)

<p>The VISN Clinical Resource Hub (CRH) Nurse Practitioner (NP) position is a clinical and administrative staff repository that serves all VISN 20 facilities. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Nurse Practitioner practices to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.</p><h3>Major Duties</h3><p>New Hire is required to live within 50 miles of a VISN 20 facility. Functions or Scope of Assigned Duties include but are not limited to the following: Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. MTE allows for the facility leader to grant employees in the identified positions an extension up to 108-days to RTO; in which case approved positions would be approved locally to grant telework/remote for those 180-days.
