ABOUT THIS ROLE
<p>The VISN Clinical Resource Hub (CRH) Nurse Practitioner (NP) position is a clinical and administrative staff repository that serves all VISN 20 facilities. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Nurse Practitioner practices to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.</p><h3>Major Duties</h3><p>New Hire is required to live within 50 miles of a VISN 20 facility. Functions or Scope of Assigned Duties include but are not limited to the following: Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. This position could be approved for Mid-Term Extensions (MTE). MTE allows for the facility leader to grant employees in the identified positions an extension up to 108-days to RTO; in which case approved positions would be approved locally to grant telework/remote for those 180-days. May be required to work on site depending on space availability (location dependent) Virtual: This is not a virtual position. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. English Language Proficiency. In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
