ABOUT THIS ROLE
<p>Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management and customer satisfaction throughout the continuum of care. Follow procedures per policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.</p><h3>Major Duties</h3><p>The Women's Health Care Coordinator executes position responsibilities that demonstrate expert leadership, experience & creative approaches in providing complex patient management. Duties are, but no limited to: Provides patient management, care coordination & discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs; which includes, but not limited to; acute, chronic, multiple, complex, catastrophic or life-threatening illnesses; combat stress, residuals or traumatic brain injury; community adjustment; addictions and other health problems. Coordinates care with multiple providers across all levels and sites of care. Assures services interface with VA and community programs to provide continuity of care to patients. Addresses psychosocial, as well as nursing & medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Coordinates and expedites care from diagnosis to treatment. Analyzes and manages laboratory and radiology results and provides information to the providers. Utilizes databases to provided population management to track patient care through all episodes of care. Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through patient/family education and provider interaction and provider interactions and implements an educational plan to meet the patient care needs. Collaborates in the development, implementation, evaluation and revision of policies, procedures and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, qualify nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, group and populations. Responsible for the documenting broad and complex outcomes at the program or service level, that can be demonstrated at any organizational level within a facility. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Note: In order to grant credit towards an appointment based on foreign nursing education higher than the associate degree level, formal degree equivalency validation from a recognized degree equivalency evaluation agency accepted by VA such as the International Consultants of Delaware (ICD) must be obtained.
