<p>The Nurse Transfer Coordinator (TC) Registered Nurse (RN) is responsible for the oversight and coordination as it pertains to patient movement and referrals to and from the facility. The nurse demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills. The TC RN demonstrates the ability to manage complex patient care for patients receiving care through all designated community and Veterans Affairs (VA) tertiary sites</p><h3>Major Duties</h3><p>Responsible and accountable for all elements of the nursing process when providing and/or supervising patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the oversight and coordination of care for patient referrals within the Veterans Health Administration (VHA), focused on patient education, self management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
