Supervisory Nurse (Case Management)

<p>About the Position: The primary purpose of this position is to exercise managerial authority over the Case Management Department (CMD) for the Medical Treatment Facility (MTF). This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD to recruit and appoint qualified candidates to positions in the competitive service.</p><h3>Major Duties</h3><p>Oversee the activities of the Case Management Department.</p><p>Responsible for identification and management of patients with targeted, high risk, and high-cost diagnoses.</p><p>Independently perform difficult and complex monitoring and evaluation of cases.</p><p>Serve as an advocate for patients, and their families, in obtaining services and support.</p><p>Perform as a first-line supervisor to the case managers and discharge planners at the core facility and collaborate with all Branch Health Clinics.</p><h3>Education</h3>FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
