ABOUT THIS ROLE

<p>The RN-Assistant Nurse Manager for Community Living Center must demonstrate specialized knowledge and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. Assesses, plans, implements, and evaluates delivery of care based on age specific components. Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>Duties include, but are not limited to: Responsible for coordination of health care for a defined resident population identified as at high risk for functional decline. Residents receive goal-oriented, coordinated care addressing holistic needs with the aim of promoting the residents' ability to attain maximum level of functioning, maintain current level of function, and/or prevent decline and promote improvement in quality of life. 1. Demonstrates a high level of clinical practice and leadership skills and is competent in conducting and reporting performance improvement indicators. 2. Demonstrates knowledge of current Long-Term Care Institute standards and application of the comprehensive MDS assessment process. 3. Demonstrates knowledge and skills necessary to provide care appropriate to the age-related needs of residents served. This knowledge includes understanding changes associated with aging and principles of growth and development relevant to the adult and geriatric population. 4. Coordinates educational activities and uses appropriate resources to develop and implement individualized care planning with Veterans and their families. 5. Provides direction to nursing staff, demonstrates professional judgement, uses problem solving techniques, and time-management principles, and delegates appropriately. 6. Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals. 7. Demonstrate effective oral and written communication skills to develop a rapport with Veterans, their families, and health team members and to ensure the fulfillment of requirements for documentation and reimbursement. 8. Acts as a resource and role model for nursing staff and students and participates in activities such as nursing committees and professional rehabilitation nursing. 9. Shares information about the disease processes, underlying disabilities and teaches nursing techniques to help Veterans and their families develop the self-care skills necessary to move toward wellness on the illness-wellness continuum. 10. Provide Veterans and their families for future self-management and decision-making responsibility by fostering Veterans' independence and goal achievement. 11. Reinforces the teaching done by specialists in rehabilitation and other healthcare disciplines and provides resource materials for Veterans changing needs. 12. Assesses the physical, psychological, socio-cultural, and spiritual dimensions of Veterans and their families, as well as their educational and discharge needs in order to formulate the restorative nursing care plan. 13. Implements a plan of care by providing nursing care and education directly or through nursing staff, as needed, to maintain and restore function and prevent complications and further loss. 14.Evaluates the nursing care that is being provided and modifies the plan, as needed, to achieve measurable goals and objectives. 15.Participates in the interdisciplinary team meetings/processes offering input into team decision making. 16.Collaborates with IDT members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the residents. 17.Collaborates with team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations. 18.Actively participates in Long-Term Care Institute (LTCI) readiness and CLC Compare performance improvement activities 19.Actively listens, reflects, and guides Veterans and their families through the stages of the grieving process to mourn the loss of abilities and roles. 20.Contributes to a safe and therapeutic environment and supports activities that promote the Veterans' return of function and prevent complication or chronic illness.
