ABOUT THIS ROLE
<p>The Veterans Affairs Oklahoma City Health Care System is seeking a Registered Nurse-Women's Health Clinical Navigator for Ambulatory Care to provide and/or supervise direct patient care. Assess, plan, implement, and evaluate delivery of care based on age specific components. Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>The 2-page Resume requirement does not apply to this occupational series. VA offers a comprehensive total rewards package: VA Nurse Total Rewards Major Duties include, but are not limited to the following: Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing. Assists Veterans, family members, and caregivers in accessing the most appropriate services and options to meet complex healthcare needs. Coordinates care with multiple providers across all levels and sites of care. Includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to Veterans. Provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the plan of care. Coordinates and facilitates, as needed, women's health care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. Interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Anticipates and implements preventive patient care measures. Provides education, guidance, and support to Veterans and caregivers, while collaborating with interdisciplinary professionals across all levels of care. Addresses psychosocial, nursing, and medical needs of Veterans and their families/caregivers through participation within the interdisciplinary care team. Notifies the Nurse Manager of any safety incidents, employee injuries, or concerns of team members not providing care according to the appropriate Standards of Care. Performs additional duties as assigned. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
