<p>The Patient Care Team (PCT) Registered Nurse (RN) Care Manager is responsible for coordinating comprehensive patient care to ensure optimal outcomes for Veterans within the VA healthcare system. The RN Care Manager works collaboratively with interdisciplinary team members, including physicians, social workers, and ancillary staff, to facilitate patient-centered care and promote health, wellness, and disease management.</p><h3>Major Duties</h3><p>Care Coordination: Coordinates care across the continuum, including primary, specialty, inpatient, and community services. Ensures seamless communication and collaboration among multidisciplinary team members. Comprehensive Patient Assessment: Conducts initial and ongoing assessments of patients' physical, psychosocial, and educational needs. Identifies Veterans at risk for deteriorating health or complex care requirements. Care Planning and Implementation: Develops individualized care plans based on assessment data, patient goals, and evidence-based practices. Implements nursing interventions to achieve desired health outcomes. Care Monitoring and Evaluation: Monitors patient responses to care, evaluates the effectiveness of interventions, and revises care plans as needed. Tracks and manages follow-up care, test results, and preventive services. Patient Advocacy and Education: Educates Veterans and their families about health conditions, treatments, self-management, and available resources. Advocates for patient needs and preferences in the care planning process. Care Transition Management: Coordinates safe, timely transitions between care settings (e.g., hospital to home, specialty referrals). Facilitates discharge planning and provides post-discharge follow-up. Population Health Management: Participates in population health initiatives, such as preventive screenings and chronic disease management programs. Analyzes and uses data to identify care gaps and improve outcomes. Documentation and Compliance: Accurately documents nursing assessments, interventions, and patient progress in the medical record. Adheres to VA policies, procedures, and regulatory requirements (including confidentiality and privacy). Quality Improvement Activities: Participates in quality improvement projects aimed at enhancing patient care and operational efficiency. Identifies trends, barriers, and opportunities for improvement.
