<p>The Home Care Registered Nurse (RN) for Spinal Cord Injury (SCI) provides comprehensive nursing care to patients within their homes, focusing on assessment, planning, implementation, and evaluation of patient care. The role emphasizes collaboration with healthcare teams to enhance the quality of life for individuals with spinal cord injuries.</p><h3>Major Duties</h3><p>Major Duties and Responsibilities: 1. Patient Care Coordination: Participate in the development and implementation of comprehensive and complex care plans for patients with spinal cord injuries, ensuring high standards of holistic and patient-centered care. 2. Collaboration and Support: Work closely with interdisciplinary teams, including physicians, social workers, and physical therapists, to coordinate care and provide comprehensive support to SCI patients in home settings. 3. Patient Education and Support: Educate patients and their families about managing their conditions, medication administration, and home health procedures to empower them in their care. 4. Clinical Expertise: Serve as a clinical resource for SCI patients, acting as an expert on nursing practices relevant to spinal cord injuries and associated complications. 5. Quality Improvement: Participate in initiatives aimed at improving patient outcomes and enhancing the standards of care for spinal cord injury patients through rigorous evaluation and adherence to best practices. 6. Resource Management: Manage home health resources effectively, ensuring the availability and utilization of medical supplies and equipment necessary for patient care. 7. Evaluation and Reporting: Utilize evaluation methods to assess the quality of nursing care provided to SCI patients, utilizing data-driven approaches to track outcomes and inform continuous improvement efforts. 8. Regulatory Compliance: Act as an advisor to ensure nursing practices exceed compliance with local, state, and federal regulations, as well as organizational policies. Participate in policy development and review. 9. Chair, organize, and coordinate the SCI Homecare Committee/Rounds 10.Cover other SCI outpatient clinic roles as needed 11.Performs other duties as assigned to meet the needs of the patients and the organization. 12. Professional Development: Stay updated on the latest practices and advancements in spinal cord injury care through continuous education, training, and professional interactions.
