Job Description:

Other duties as assigned. Communicates with patients and families after discharge to address concerns and ensure understanding of follow-up care and functional assessment. Collaborates with the multidisciplinary team to ensure high-quality patient care. Participates in community education as needed. Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner. Provides telephone and face-to-face consultations with patients, answering questions, providing educational information, and reviewing discharge information. Navigates patients from the diagnostic phase through treatment, ensuring timely medical and supportive services, including responding to stroke alerts and transfers. Coordinates patient education and care, serving as a resource for patients and families during diagnosis and treatment, and communicating patient status and needs. Collaborates with the Neuroscience Institute department to enhance patient care. Informs and seeks assistance from the Manager and Director when confronted with unfamiliar procedures and techniques to ensure safe and effective patient care. Develops, plans, and implements evidence-based measures to improve the quality of stroke patient care and ensures compliance with regulatory guidelines. Supports and assists with data collection for process and quality improvement, understanding department performance trends and standards.Knowledge, Skills, and Abilities:

• Effective verbal and written communication skills [Required]

• Ability to read, write and explain moderately complex information to hospital staff with a great deal of attention to detail [Required]

• Ability to communicate effectively in a supportive manner to patients, family, staff and physicians [Required]

• Ability to explain information to patients and families in a way the patient understands, and/or explain departmental or hospital procedures to employees, visitors and patients [Required]

• Ability to observe assess, guide and evaluate others [Required]

• Ability to communicate timely and in an open and honest manner [Required]

• Ability to collect data, interpret findings, set priorities and carry out established plans [Required]

• Flexibility in prioritization; ability to multitask; critical thinking/problem-solving skills [Required]

• Self-motivated with ability to work independently and in a group setting [Required]

• Strong organizational skills [Required]

• Computer skills such as Microsoft Word, Access, Excel and Outlook [Preferred]

• Knowledge of PACS, Apollo and associated systems [Preferred]

• Knowledge of American Heart Association/American Stroke Association Stroke Guidelines [Preferred]

• Knowledge related to telemedicine practice [Preferred]

• Strong communication, analytical, interpersonal, and writing skills [Preferred]

• Ability to work proactively and handle multiple tasks simultaneously [Preferred]

Education:

• Associate's of Nursing [Required]

• Bachelor's of Nursing [Preferred]

Field of Study:

• N/A

Work Experience:

• 2+ years critical care and/or emergency care setting [Preferred]

• Previous experience with projects or program start-up [Preferred]

• Previous leadership experience [Preferred]

Licenses and Certifications:

• Registered Nurse (RN) [Required]

• Basic Life Support - CPR Cert (BLS) [Required]

• Advanced Cardiac Life Support Cert (ACLS) [Preferred]

Pay Range:

$32.76 - $57.47
