ABOUT THIS ROLE
<p>This position is located at the Clement J. Zablocki VA Medical Center in Milwaukee, Wisconsin (http://www.milwaukee.va.gov/). The Patient Aligned Care Team (PACT) Outpatient Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.</p><h3>Major Duties</h3><p>The PACT RN assumes responsibility for the coordination of care focused on the patient education, self-management, and customer satisfaction throughout the continuum of care. The PACT RN administers medications and procedures per established policies and guidelines. The PACT RN influences care outcomes by collaborating with members of the interdisciplinary team. The PACT RN duties include, but are not limited to: Knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Responsible for providing competent, evidence-based care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriated to the setting. Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Provides sex-specific health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement and fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Able to share educational findings, experiences, and ideas with peers. Participates in activities and strategies to sustain evidence-based-practice.
