ABOUT THIS ROLE
<p>The Referral Clinical Coordinator at the Sioux Falls Veterans Affairs Healthcare System (SFVAHCS) Community Care (CC) serves as a vital member of the nursing team and operates independently with a high level of initiative, innovation, and resourcefulness. This role involves the coordination of episodic care for Veterans seeking treatment with community providers or urgently needing referrals for community services.</p><h3>Major Duties</h3><p>Major duties include, but are not limited to: Interacts cohesively with Veterans, family members, and interdisciplinary teams to provide Veteran-centered services that ensure well-informed decisions, enhance satisfaction, and efficiently deliver quality care. Perform case management, direct communication with interdisciplinary teams, chronic disease management, resource monitoring to contain costs, and utilization of technology for clinical triage. Advocate for Veterans by supporting decisions, clarifying options, and problem-solving with the interdisciplinary team, ensuring a seamless transition back into the VA system. Utilizes all available resources to provide safe, efficient, and timely coordination of health care services, demonstrating leadership, creativity, and a commitment to the Veteran's well-being while supporting the SFVAHCS mission and customer service principles in all aspects of work. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
