<p>At Sioux Falls VA Health Care System, Certified Foot Care Nurse (CFCN) Registered Nurse (RN) care managers partner with Veterans and families to coordinate care and ensure accurate documentation-empowering informed decisions and timely, high-quality services across settings.</p><h3>Major Duties</h3><p>The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Working cohesively with Veterans, family members, and an interdisciplinary team of professionals. Applying knowledge of care coordination guidelines and documentation requirements; ensuring accuracy in the electronic health record. Providing Veteran-centered services that enable best-informed decisions, increase satisfaction, and support quality care in a timely manner. Collaborating to promote positive clinical, financial, and psychosocial outcomes for a caseload of Veterans with varied chronic and acute diagnoses. Demonstrate knowledge and skills in foot care, including, but not limited to: toenail care (including use of Dremel), basic and intermediate foot care, a variety of wound dressing changes related to co-morbidities and risk factors, identify potential for skin breakdown and intervene with appropriate preventative measures, collect pertinent data, ongoing monitoring and documentation including pressure, surgical, vascular, and diabetic/neuropathy wounds, and use of nurse protocols. Participating in performance improvement; identifying opportunities to improve patient care through monitoring, analyzing, and evaluating care outcomes. Exhibit leadership in assisting other staff to assess and evaluate patients foot skin integrity and wounds. Communicating effectively with internal and external customers and collaborating with facility and Community-Based Outpatient Clinics to ensure continuity of care. Permanent Change of Station (PCS): Not authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
