<p>The Registered Nurse will be assigned to a Patient Aligned Care Team within the Primary Care Department at the Cape Girardeau Health Care Center.</p><h3>Major Duties</h3><p>General Description of Assigned Duties: The Registered Nurse within the Patient Aligned Care Team is responsible for and accountable for all aspects of the nursing process when delivering and supervising direct patient care. This role includes coordinating care with a focus on patient education, and ensuring high levels of patient satisfaction throughout the entire continuum of care. The nurse administers medications and procedures in accordance with established policies and guidelines. Additionally, the nurse contributes to positive care outcomes by collaborating effectively with interdisciplinary team members. Key performance responsibilities include maintaining knowledge of and actively participating in unit or program quality improvement initiatives, as well as supporting customer service excellence. Functions and Scope of Assigned Duties: The Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for delivering competent, evidence-based, gender-specific care to primary care patients. This role involves providing direct veteran care and care coordination within the Primary Care Department. The nurse manages assigned patient panels, utilizes registries effectively, and ensures appropriate evaluation and access to care for these patients. Responsibilities include health promotion, health education, and coaching on wellness, disease prevention, and chronic disease management. The nurse supports the delivery of nursing education, orientation, and competency development, as well as contributing to quality improvement initiatives and outcomes analysis. Demonstrating leadership, the nurse collaborates with team members to enhance holistic, primary care services. They continuously evaluate practice based on current best evidence, provide constructive feedback to peers, and offer professional perspectives in interdisciplinary team discussions. The nurse partners with colleagues to facilitate positive change and achieve optimal patient outcomes. Preferred Experience: 2-3 years of clinical experience is preferred. Experience in Care Management is a plus.
