ABOUT THIS ROLE

<p>The Home Based Primary Care (HBPC) program delivers comprehensive, interdisciplinary primary care directly in the homes of Veterans who face significant challenges traveling to clinic appointments. These Veterans often have complex, chronic illnesses, are homebound or isolated, or rely on caregivers. HBPC allows them to receive high-quality care in familiar, comfortable surroundings. The Nurse makes a meaningful difference helping Veterans achieve lifelong health and wellness where they live.</p><h3>Major Duties</h3><p>ROLE RESPONSIBILITIES AND ACCOUNTABILITIES BUT ARE NOT LIMITED TO THE FOLLOWING: As an HBPC Registered Nurse, you will serve as the care manager for Veterans in the St. Joseph, Mo. region and surrounding areas. In this highly rewarding role, you will: Provides full range of nursing services utilizing the nursing process. Provide direct nursing care in the home, including but not limited to: wound care, blood draws, medication setup, and foley catheter care. Assess the needs of Veterans and caregivers while coordinating services across a multidisciplinary team. Deliver education focused on chronic disease management, reducing hospitalizations, and promoting independence. Build strong, supportive relationships with Veterans and their families. Work independently in the community, requiring excellent organization, communication skills, and a valid driver's license. Most of the RN's time will be spent in the community caring for Veterans in their homes. A government car is provided available for travel to Veteran's homes. Administers prescribed therapies using nursing judgement. Promotes discharge planning and follow up care as needed. Listens to and includes patient and family in plan of care. Provide feedback and patient education. Communicates with Veterans on their health care options including local VA, other VA medical centers and community-based eligibility. Works closely with other VA Medical Centers, contracted facilities, and community facilities to remove obstacles that would impede the Veteran from utilizing the services of other VA Medical Centers. Educates providers on medical management processes. Provides clinical knowledge and acts as a clinical resource to non-clinical multidisciplinary staff. Other duties assigned. Preferred Experience: Home Care, Acute Care, ER/Critical Care, Primary Care. Permanent Change of Station (PCS): Not Authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
