<p>The Patient Aligned Care Team - Outpatient Staff Registered Nurse delivers knowledge-based care, performs assessments, plans and evaluates treatment, coordinates PACT panel activities, and supports health promotion, disease prevention, and chronic care management while collaborating to provide Veteran-centered care at the Austin VA Outpatient Clinic.</p><h3>Major Duties</h3><p>CTVHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care. This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care. This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being. All staff in this facility will be part of this process, shifting the focus from the question, "What's the matter with you?" to the question, "What matters to you, and how can we help you live your best life?" This strategy also involves integrating a full range of conventional approaches with Complementary and Integrative Health (CIH) approaches in order to optimize lifelong health and well-being, as defined by the Veteran, rather than narrowly focusing on disease management. The Patient Aligned Care team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. Provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panels(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Provides health promotion, health education and coaching on wellness, disease prevention, and chronic are management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for the Primary Care department and facility. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professionals. Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations. Responsible for the documented PACT outcomes at the program or service level. Complete all application requirements detailed in the "Required Documents" section of this announcement.<h3>Education</h3>Education Accreditation: The Associate Degree in Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science Degree in Nursing (MSN) must be recognized by the appropriate State-accrediting agency and accredited by either of the following accrediting agencies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) OR The Commission on Collegiate Nursing Education (CCNE) ACEN or CCNE program or degree accreditation may also be found under the U.S. Department of Education Notes: Department of Education: Only education or degrees recognized by the U.S. Department of Education from accredited colleges, universities, schools, or institutions may be used to qualify for Federal employment.
