ABOUT THIS ROLE
<p>The Registered Nurse - Patient Aligned Care Team (PACT) Care Manager delivers and improves holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned.</p><h3>Major Duties</h3><p>The Registered Nurse (RN) - Patient Aligned Care Team (PACT) Care Manager works in partnership with an assigned panel of patients and team members for effective disease and chronic care management. They use the nursing process and evidence-based practice to collaborate with the expanded team, internal and community-based services, in developing the patient driven care plan for prevention and management of diseases. Major Duties include, but are not limited to: Responsible for collection of data and implementing evidence based changes to practice based on data results. Collaborates with the services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs. Provides evidence-based patient and family health education with a focus on self-management, prevention, and wellness. Initiates follow-ups as recommended while administering the prescribed therapies using appropriate judgement. All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service. Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
