ABOUT THIS ROLE

<p>Home & Community Based Care (H&CBC), Hospital in Home Program (HIH) provides care for recently "early discharged", stable Veterans requiring acute hospital-level care and services after discharge. The HIH program assists the patient/family/caregiver in developing appropriate plans to guide them through immediate medical changes that have occurred. Incumbent of this position delivers fundamental knowledge-based care to assigned clients while developing technical skills in the veteran's home.</p><h3>Major Duties</h3><p>The nurse assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and perform procedures in the home environment. Influences care outcomes by collaborating with members of the interdisciplinary team. Duties include but are not limited to: Working independently to perform home visits for recently discharged clients; Performs thorough and focused assessment Demonstrating sound judgment in decision-making; Assessing and identifying medical and caregiver needs to provide appropriate referrals; Documenting visits in the electronic patient record within program guidelines; Reviewing care issues with appropriate disciplines; Administers medications and procedures per established policies and guidelines. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). Pre-Employment/Post-Employment Physical Examination: May be required, depending on the needs of the organization<h3>Education</h3>The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE). Verification of accreditation may be obtained from http://www.acenursing.us/accreditedprograms/programsearch.htm or https://directory.ccnecommunity.org/reports/accprog.asp If your school has changed names, or is no longer in existence, you must provide this information in your application. If your school was accredited by ACEN or CCNE at the time of graduation, but is no longer listed on the websites, you must provide documentation of accreditation with your application packet. If you are using foreign education to meet qualification requirements, you must send a Certificate of Foreign Equivalency with your transcript in order to receive credit for that education.
