Registered Nurse (Outpatient - Home & Community-Based Care)

<p>Home & Community Based Care (H&CBC), Hospital in Home Program (HIH) provides care for recently "early discharged", stable Veterans requiring acute hospital-level care and services after discharge. The HIH program assists the patient/family/caregiver in developing appropriate plans to guide them through immediate medical changes that have occurred. Incumbent of this position delivers fundamental knowledge-based care to assigned clients while developing technical skills in the veteran's home.</p><h3>Major Duties</h3><p>The nurse assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and perform procedures in the home environment. Influences care outcomes by collaborating with members of the interdisciplinary team. Duties include but are not limited to: Working independently to perform home visits for recently discharged clients; Performs thorough and focused assessment Demonstrating sound judgment in decision-making; Assessing and identifying medical and caregiver needs to provide appropriate referrals; Documenting visits in the electronic patient record within program guidelines; Reviewing care issues with appropriate disciplines; Administers medications and procedures per established policies and guidelines.
