<p>At the U.S. Department of Veterans Affairs (VA) Sioux Falls VA Health Care System, Registered Nurses (RN) in Home Telehealth coordinate technology-supported care from enrollment through follow-up-assessing needs, educating families, and responding to clinical alerts-to strengthen continuity and outcomes for Veterans across settings.</p><h3>Major Duties</h3><p>The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Managing all aspects of Home Telehealth consults per program policies, including receiving/processing consults and enrolling/disenrolling patients. Issuing, receiving, and documenting telehealth equipment; determining and reassessing the appropriate category of care, including evaluation for Non-Institutional Care services. Completing comprehensive initial and ongoing assessments of patient conditions, treatment responses, and clinical needs; initiating follow-up as appropriate. Assessing learning needs; developing and implementing individualized education plans; providing instruction to patients and families tailored to readiness and comprehension. Promoting continuity by collaborating with patients, caregivers, and the interdisciplinary team to develop and coordinate individualized plans of care. Conducting phone or video telehealth appointments and providing technical assistance; applying evidence-based practice and technology-supported education to support disease management and reduce unnecessary health care utilization. 1 Monitoring clinical alerts and response rates; intervening appropriately; documenting assessments, actions, and outcomes in the Electronic Health Record (EHR). Ensuring compliance with Office of Connected Care Conditions of Participation and adhering to infection control and equipment safety standards. Collaborating with facility and community-based outpatient clinic teams to support seamless care across settings; participating in performance improvement and recommending innovative solutions to improve outcomes. Engaging in ongoing professional development through self-assessment, evaluation, feedback, and educational activities; demonstrating professional behavior and effective communication. Permanent Change of Station (PCS): Not authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
