<p>The VA Northeast Ohio Healthcare System is recruiting for an Registered Nurse (Collaborative Care Case Manager). The Registered Nurse will function in the Patient Transfer Center (PTC) section of the Patient Care Administrative Service (PCAS).</p><h3>Major Duties</h3><p>Duties of the Collaborative Care Case Manager include but are not limited to: Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial, as well as coordinating home-going nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluates care and outcomes to ensure timely and appropriate provision of services. Brings principles and theory with limited application of critical thinking skills. Utilizes National Management Advisory Committee (NUMAC) approved utilization management criteria and software applications for the purpose of collecting, analyzing and reporting under/over utilization of medical center resources, systems barriers to throughput and other activities as assigned to improve patient care and organizational processes. Responsible for providing competent, evidence-based care to patients. Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. Demonstrate the ability to cope with and manage competing priorities. Executes position responsibilities that demonstrate leadership, experience, and creative approaches in coordinating and overseeing complex patient management. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through CCCM knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Participates in activities and strategies to sustain an evidence-based-practice culture. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
