ABOUT THIS ROLE
<p>The Intermittent Registered Nurse - Cardiology Care Coordinator position is located within the Nursing Specialty Service Line with the NWI VA Health Care System, Omaha, NE.</p><h3>Major Duties</h3><p>The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. The duties of the Registered Nurse - Cardiology Care Coordinator include, but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Responsible for providing competent, evidence-based care to a specific cardiology specialty population. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Coordinates and expedites care from diagnosis to treatment. Analyzes and manages laboratory and radiology results and provides information to the providers. Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs. Provides clinical support for the cardiac and peripheral interventional, heart failure, cardiothoracic/TAVR, pacemaker / ICD clinics and General Cardiology Clinics in collaboration with the Cardiology team. Interacts as part of the inpatient and outpatient multidisciplinary care team including management of the patient from the inpatient to outpatient transition in care. Collaborates with the Cardiology team including Physicians, Extended Providers, other RNs, Licensed Practical Nurses (LPNs), Sonographers and Radiologic Technologists. Work Schedule: Intermittent, on an as needed basis: 0730-1600 Monday - Friday (depending on clinical need). Telework: Not Available Virtual: This is not a virtual position.
