ABOUT THIS ROLE
<p>The Admission/Discharge Coordinator, Registered Nurse, is an active member of an interdisciplinary team whose primary focus is on care transitions to and from an inpatient setting.</p><h3>Major Duties</h3><p>The Admission/Discharge Coordinator, Registered Nurse, duties include, but are not limited to the following: Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on aged-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient in and out of the acute inpatient setting. Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes. Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure patients needs are addressed. Seeks guidance in how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, safety of the patient. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
