<p>The Cardiology Advanced Practice Nurse (APN) - Certified Nurse Practitioner (NP) serves within the National CARDIO-VET Network, delivering a comprehensive range of cardiology care aligned with the mission of the Department of Veterans Affairs. The APN provides advanced clinical practice, expert consultation, and ongoing patient management while ensuring high-quality, timely, and Veteran-centered services.</p><h3>Major Duties</h3><p>Major duties and responsibilities of the position VN-0610-II and VN-0610-III include but not limited to: Independently manages a panel of patients with cardiovascular conditions with an emphasis on cardiovascular disease management using evidence-based practice. Develops and implements an individualized plan to achieve optimal cardiovascular care management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
