<p>Excited to Announce Salem VA Medical Center is looking for a Registered Nurse (RN) Referral Coordinator. Demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines.</p><h3>Major Duties</h3><p>The Registered Nurse Referral Coordination Team (Referral Coordination Team ) Registered Nurse (Registered Nurse) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. The Referral Coordination Team Registered Nurse possesses intimate knowledge of the inteRegistered Nurseal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines. Duties listed below but not limited to: Responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Registered Nurse serves as a liaison to both inteRegistered Nurseal and community providers and is responsible for managing the care of the Veteran throughout the consult process. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Referral Coordination Team Registered Nurse will triage incoming referrals, discuss available care options, provide scheduling guidance, and be a resource within the service. Referral Coordination Team Registered Nurse will guide Veterans through their full range of care options including direct care in VA and care in the community utilizing a national standardized triage tool. Referral Coordination Team s serve as the liaison between ordering or referring VA providers and specialty care services by clinically triaging consults or referrals to confirm the appropriate care is requested and all necessary pre-work and tests are completed. Referral Coordination Team Registered Nurse will comply with VA-mandated use of health information solutions that serve to store, exchange, and track referral care information. Referral Coordination Team nurse will work collaboratively with the Office of Integrated Veteran Care to triage Request for Services (RFS) to promote inhouse service access. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
