ABOUT THIS ROLE
<p>The Registered Nurse delivers fundamental knowledge-based nursing care to Veterans in the Specialty Care Outpatient setting. Responsibilities include assessing, planning, implementing, and evaluating care; coordinating care with the interdisciplinary team; administering medications and treatments; and providing patient education to promote optimal health outcomes.</p><h3>Major Duties</h3><p>The Hampton VA Health Care System is seeking a Registered Nurse (RN) to join the Specialty Care Outpatient Nursing team. The RN delivers comprehensive, Veteran-centered nursing care in a multidisciplinary outpatient setting while promoting quality, safety, and exceptional patient outcomes. The Hampton VA Health Care System is a Level 1C teaching facility serving Veterans throughout Eastern Virginia and Northeastern North Carolina. The health care system includes the Hampton VA Medical Center and community-based outpatient clinics located in Chesapeake, Portsmouth, Virginia Beach, and Elizabeth City, offering comprehensive primary care, specialty care, mental health, inpatient services, spinal cord injury care, community living, hospice, and other healthcare services. Located in the Hampton Roads region of coastal Virginia, Hampton offers a high quality of life with waterfront living, historic attractions, excellent outdoor recreation, and convenient access to Virginia Beach, Norfolk, Williamsburg, and Richmond. Major Duties include, but are not limited to: Assess, plan, implement, coordinate, and evaluate individualized nursing care for Veterans in an outpatient specialty care setting. Perform patient triage, including assessment of walk-in patients, and collaborate with providers to determine appropriate plans of care. Coordinate care across the continuum through collaboration with physicians and the interdisciplinary healthcare team. Provide patient and family education related to disease management, medications, preventive care, and healthy lifestyle practices. Administer medications, intravenous therapies, hydration, blood products, biologics, and other prescribed treatments in accordance with VA policies and standards of practice. Assist providers during specialty procedures and perform treatments within scope of practice. Conduct nurse visits, monitor patient progress, and accurately document care in the electronic health record. Participate in daily huddles, schedule management, clinic readiness activities, accreditation preparation, and Environment of Care initiatives. Promote evidence-based practice, patient safety, infection prevention, and continuous quality improvement. Maintain effective communication with Veterans, families, providers, and members of the interdisciplinary team to ensure coordinated, patient-centered care. Demonstrate professionalism, accountability, and adherence to VA nursing standards while providing exceptional customer service. Perform other related duties as assigned.
