<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Technically proficient in initiating, performing, and completing assigned duties in providing care to variable patient populations. Knowledge and ability to appropriately carry out assigned patient care based on the patents' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information. The incumbent will serve as a patient site clinical coordinator of telehealth services in assigned clinics. Is essential to the clinical aspects of the intervention. Is available for all telehealth visits to carry out telehealth duties and is responsible for the patient site clinic: business clinical and technical aspects of synchronous and asynchronous telehealth applications including all specialty and primary care telehealth encounters. Performs telehealth imaging including but not limited to tele-retinal screening, tele-dermatology tele-wound care imaging and other telehealth programming as technology becomes available and as identified and referred by the ordering/referring provider. The LPN TCT can initiate consultations for patients on behalf of the primary care or specialty provider who retains responsibility for patient care. This includes but is not limited to telehealth encounters for all specialty and primary care clinics. The LPN-TCT reviews and maintains telehealth supply inventories. Train staff on all video conferencing technologies, telehealth peripherals including stethoscopes, otoscopes, ultrasound Doppler technology, and scheduling software used to coordinate manage telehealth resources and connect staff patients and providers. The LPN-TCT maintains effective telehealth delivery of services through patient care, education, and training. Completed work at this level should need only a general review by the registered nurse or provider for appropriateness and conformity with established policy and procedures. 1. Responsible for observing recording and reporting changes in patient condition as well as assisting in the development and revision of a patient driven holistic care plan for life. 2. Serves as a generalist role and presenter to support synchronous telehealth, asynchronous telehealth, staff training, equipment management and systems. Continues to support telehealth clinical encounters from the patient location as the site telehealth LPN this includes patient education activities, technical support and training. Provides completion of business processes, administrative support, and other program support duties as needed. 3. Demonstrates active collaboration and communication with the patient, facility telehealth coordinator and patient aligned care team (pact) involved in providing care. 4. The LPN TCT may perform intraocular pressure, dilation, and visual field assessment. Performs other duties as assigned.
