Practical Nurse - Patient Aligned Care Team

<p>This announcement is for a Practical Nurse in Gaylord, MI. The primary purpose of the LPN is to perform assigned duties in a manner that promotes quality health care, positive customer service and maintenance of an environment committed to serving the Veteran.</p><h3>Major Duties</h3><p>Total Rewards of a Allied Health Professional Duties included but are not limited to: General duties: Assist with triage as appropriate. Assist with exams/procedures. Assist with group visits as directed. Set up supplies, prep room for procedures. Perform treatments (VS, EKG, FSBG, ear lavage, toenail care) Administer meds, performs wound care. Obtains and records objective and subjective data relevant to a historical biopsychosocial profile (e.g., temperature, pulse, respirations, blood pressure, I & O). Refers patients to appropriate resources (dietary, social work, OIF/OEF, smoking cessations, etc.) after screening/reminders completed. Initiates and implements measures to ensure a clean and safe environment for patients and staff. Provides patient privacy. Consistently demonstrates professional and responsible behavior (i.e., punctuality, neat appearance, code of conduct is followed). Documents patient care in the medical record according to Patient Care Management guidelines. Documentation is comprehensive and reflects current patient status. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
