ABOUT THIS ROLE
<p>The John D. Dingell VA Medical Center is recruiting for a Nurse Practitioner- Palliative Care. The NP practices to provide a full range of services in outpatient/inpatient settings emphasizing quality, timeliness & the advancement of the mission. The NP provides advanced clinical practice, consultation & management. The NP practices to the full extent of their education, training, & certification.</p><h3>Major Duties</h3><p>The NP has prescriptive authority for a panel of patients or program as indicated by their position or assigned by their supervisor. The major duties include, but are not limited to: Independently manages or in collaboration with attending physician(s), a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Initiates referrals and consults to specialty services. Provide End-Of-Life care and symptom management to all patients in an inpatient and outpatient setting. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Provider is committed to delivering compassionate, timely and Veteran-centered care through multidisciplinary palliative care teams. These teams support Veterans in VA hospitals, community living centers (CLCs) and even remotely via telehealth services. Provides medical services to veterans on an Outpatient, Inpatient or assigned by the supervisor as needed at the John D Dingell VAMC under GEC service.
