<p>Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Follows procedures per established polices and guidelines.</p><h3>Major Duties</h3><p>The functions and duties of the NURSE MANAGER - CLC AND HOSPICE include but are not limited to the following: Nurse Manager (NM) is the designated first-line supervisory authority and is responsible and accountable for the 24/7 operation of their assigned units and provides coverage for other NMs as needed. The NM is responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance. They enhance care outcomes by collaborating with members of the interdisciplinary team to remove barriers and implement new evidence-based cutting-edge programs/processes. They identify safety concerns and hazardous items to maintain a safe and therapeutic environment of care, and documents nursing assessments, nursing plan of care, patient safety observations, and environmental safety rounds in accordance with facility policy. The NM participates in the development of care delivery models and environmental design, ensuring evidence-based, accessible, affordable, and equitable patient care. The NM within Nursing Geriatric Extended Care (GEC) demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The NM will be responsible for maintaining continuous accreditation readiness of their units, including adherence to Ascellon, Office of Inspector General, Centers for Medicare and Medicaid Service (CMS) Standards and other CLC and Hospice metrics and other external and internal accreditation and regulatory standards as indicated. They will work in concert with the Chief Nurse and Assistant Chief Nurse to arrange for managerial coverage to handle administrative and clinical issues in their absence. Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans. Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
