Nurse (Community Health)

<p>This position is located at a Service Unit within the Indian Health Service (IHS). The primary purpose of this position is to provide professional community/public health nursing services through direct care to individuals, families, and identified populations through direct care, home and school visiting, and population-based assessment, prevention, and intervention activities. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).</p><h3>Major Duties</h3><p>Assesses the health needs of individuals, families, and the community served, taking into account the total situation in the family, the home, and in the community environment. Develop individualized and family-centered care plans. Implements and evaluates plans of care to support prevention, early intervention, treatment adherence, and continuity of care. Shares health information and collaborates with patients, families and other community resources. Educates and coaches people to make appropriate and informed decisions about health issues. Apply population health assessment and surveillance findings to support targeted prevention, early intervention, and public health nursing activities within the assigned service unit.
