<p>VA Greater Los Angeles Healthcare System is seeking to hire an experienced Licensed Vocational Nurse (LVN) to work in our Case Management/Care Coordination Program. The LVN provides patient centered care with expertise in the coordination of care for high-risk clients in the outpatient/inpatient setting ensuring that Veteran-centered health care is provided and delivered in a quality, accessible and respectful manner, consistent with the mission, goals and objectives of the organization.</p><h3>Major Duties</h3><p>This job announcement is open to current, permanent GLA employees only. Duties and responsibilities, including but not limited to: Performs more complex nursing assignments within the practice setting. Demonstrates broader knowledge skills with respect to responsibilities than employees at lower grades. Applies Veteran's care data to initiate active contributions to the Veteran's overall health plan. Demonstrates the ability to work independently without the need for readily available supervision and communicates effectively with the Veteran, families, caregivers, professionals, and other supportive staff. Collaborates with Surgical Services, Procedure Areas, Primary Care, and specialty teams to ensure timely communication of abnormal findings, incomplete requirements, or changes in patient condition that may impact surgical scheduling. Monitors and documents patient completion of pre-procedure steps, notifying RN/provider of barriers, delays, or clinical concerns requiring intervention. Assists with post-procedure coordination such as scheduling follow-up appointments, reinforcing discharge instructions, reviewing wound and skin integrity, and identifying potential complications requiring RN/provider attention. Demonstrates ability to recognize early signs of complications related to surgery or procedures and promptly reports concerns to the RN/provider for further evaluation. Ensures accurate and timely documentation of all coordination activities, patient instructions, assessments, and communications in CPRS according to policy. Participates in interdisciplinary team huddles and care coordination meetings as requested to support safe surgical workflows and patient transitions. Assists patients and families in understanding pre-procedure requirements and actively contributes to reducing same-day cancellations by promoting readiness and adherence to instructions. Assists the RN in assessing social, logistical, or functional needs that may impact surgical preparation or discharge planning (e.g., transportation, caregiver support, home safety). Supports the organization's focus on safe, efficient care delivery by identifying system barriers, communicating delays, and collaborating to resolve issues affecting timely surgical access. Demonstrates competency in navigating scheduling systems, reviewing surgical checklists, and coordinating required components of the perioperative process. Facilitates procurement of patient records. Collaborates with Care in the community. Provides care coordination across assigned services and programs, including assisting with inpatient care coordination to support safe, effective transitions of care. Demonstrates proficiency in developing, maintaining, and interpreting materials using Microsoft Excel, Word, PowerPoint, and Power BI. Utilizes these tools to support VA performance management, including trending, tracking, and reporting key data elements. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
