<p>A Licensed Practical Nurse at the Loch Raven VA Medical Center serves as a member of the multidisciplinary health care team with selected patients and provides care to them on a regular and recurring basis.</p><h3>Major Duties</h3><p>The Licensed Practical Nurse serves as a member of the multidisciplinary health care team with long and short stay resident care and provides care to them on a consistent and recurring basis. The incumbent will prepare and administer prescribed medications and perform treatments according to established policies/procedure. He/she will give complete personal and hygienic care. The Licensed Practical Nurse will also provide assistance with food and fluid intake, recording of I&O's; vital signs, medications, treatments, and patient care. Excellent customer service and communication skills are required. Adheres to computerized patient record system policies and procedures for administration and data entry for vital signs and other patient information. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD. Serves as a preceptor in orienting, educating and training less experienced LPN's or nurse assistants/health care technicians related to support duties for these more complex, specialized tests/procedures. Adaptable to the changing needs of the medical center; accepts changes in work assignments readily, recognizing that changes are often needed in providing patient care. *This position is located at the Loch Raven VA Medical Center* Preferred Experience: 2 years Long Term Care experience; preferred.
