<p>This position is located in the Primary Care located at the VA Southern Oregon Healthcare System, White City VA Medical Center, White City OR.</p><h3>Major Duties</h3><p>The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of 3 to 6. Duties of this position include but are not limited to the following: Provides ongoing, continuous care of one panel of Veterans. Ensures planning, assessment and appropriate evaluation and access is provided to Veterans assigned to the PACT panel. Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to Veterans. Manages communications and facilitates safe transitions of Veterans between the PACT's site of care and other health care settings, using informal and formal communication methods as appropriate. Provides health education skills to Veterans and personal support persons, using Health Promotion and Disease Prevention resources. Engages Veterans in using health care, encouraging them to engage personal support persons, and listens to Veterans and personal support persons regarding their VA care. Uses formal and informal communications that are respectful, effective, timely, and bidirectional with all team members (including the patient and personal support persons) to convey significant, clinically relevant information for the care of the Veteran. Safeguard the confidential information acquired from any source about the patient. Provide health care to all patients regardless of race, creed, cultural background, disease, or lifestyle. Uphold the highest standards in personal appearance, language, dress, and demeanor. Stay informed about issues affecting the practice of nursing and delivery of health care and, were appropriate, participate in government and policy decisions. Accept the responsibility for safe nursing by keeping oneself mentally fit, physically able, and educationally prepared to practice. Know the scope of nursing practice authorized by the Nursing Practice Act in the state wherein employed. Have a personal commitment to fulfill the legal responsibilities inherent in good nursing practice. Take responsible actions in situations wherein there is unprofessional conduct by a peer or other health care provider. Recognize and have a commitment to meet the ethical and moral obligations of the practice of nursing. Accept assigned responsibilities as an accountable member of the health care team. Function within the limits of educational preparation and experience as related to the assigned duties. Function with other members of the health care team in promoting and maintaining health, preventing disease and disability, caring for and rehabilitating individuals who are experiencing an altered health state, and contributing to the ultimate quality of life until death.
