<p>Delivers fundamental knowledge-based care to assigned clients while developing technical skills, Assesses, plans, implements, and evaluates care based on aga-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.</p><h3>Major Duties</h3><p>Duties and responsibilities of the COMMUNITY CARE NETWORK (CCN) CARE MANAGER REGISTERED NURSE include, but are not limited to: Community Care Network (CCN) Care Manager, is a registered nurse (RN), who executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within VHA and VA-Community Care program. RN has the primary responsibility for providing clinical expertise in coordinating Veteran care under the care for Short Term Rehab (30, 60, 90 and up to 100 calendar days) in a Skilled Nursing Facility, in the community for planned and unplanned/emergent episodes of care, and discharge disposition needs. This includes receipt of initial requests for transfers from community hospitals to skilled nursing facilities under OPTUM. The RN provides close monitoring of continued stay reviews, maintaining nursing standards across service lines, as well as being able to multitask, be flexible and adaptable in the evolving community care environment. The CCN will collaborate with the Veteran, family, the primary care provider (PCP), the specialty care provider (SCP), the payor, and all other service professionals and paraprofessionals. The CCN will maintain direct, open, and honest communication while adhering to the Veteran's rights of privacy and confidentiality. The RN will evaluate patient safety and satisfaction, participate in performance improvement projects and monitor data to determine effectiveness of care. The CCN spends substantial time communicating and educating. Therefore, it is vital to possess the skills to communicate clearly and concisely, both orally and in writing. Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans. Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
