Job Description:

*Schedule: Monday-Friday 8:00am-5:00pm *

• Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.
• Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space. Communicates through various digital channels to support patient care.
• Performs initial health screens and intake for new members. Obtains and reviews old medical records to ensure comprehensive patient history.
• Identifies care gaps and orders necessary screening tests. Confirms appointments and provides instructions to patients.
• Conducts pre-visit RAF reviews to prepare for patient visits. Welcomes patients, provides tours of the practice, and reviews welcome packets. Ensures all medical records are available for office visits.

• Knowledge of chronic disease management [Required]

• Empathetic nature and a distinct ability to consistently provide top tier service. [Required]

• Knowledgeable in member advocacy and navigating complex systems. [Required]

• Responsible for managing the member’s entire care plan with the guidance of the RN Case Manager and Provider. [Required]

Education:

• Technical/Vocational School [Required]

Field of Study:

• Graduate of a Medical Assistant Program

• Basic Life Support - CPR Cert (BLS) [Required]

• Certified Medical Assistant (CMA) [Required] OR

• Clinical Medical Assistant Certification (CMAC) [Required] OR

• Registered Medical Assistant (RMA) [Required] OR

• Certified Clinical Medical Assistant (CCMA) [Required] OR

• National Certified Medical Assistant (NCMA) [Required] OR

• Nationally Registered Certified Medical Assistant (NRCMA) [Required]
