<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>This Licensed Practical/License Vocational Nurse (LPN/LVN) position is located in the Nursing and Ambulatory Care service line at the Manchester New Hampshire VA Medical Center. The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran. Performs under guidance of the Nurse Manager or designee. Major Duties include, but are not limited to: Serves as an LPN on a Patient Aligned Care Team (PACT) in Primary Care. Assists with intake of Veterans in preparation for provider visits and maintains clinic flow. Completes pertinent clinical reminders and preventative health screenings. Documents all data collected and nursing care provided in the Veterans electronic medical record. Reports all relevant patient information to Clinic RN or provider. Prioritizes effectively and manages time productively in the clinical setting. Works effectively with others and communicates in an appropriate, tactful and courteous manner. Assists providers with exams/procedures as needed. Performs treatments and procedures such as electrocardiograms, vital signs, weights, blood glucose readings, basic foot care, bladder scans, ear flushes, etc. Tracks and administers required immunizations. Administers ordered medications in accordance with Medical Center and Nursing Service policy and monitors for any side effects. Participates in the management and follow up of patient care needs and requests. Initiates procedure preparations, adapting them to patients physical and emotional needs before, during and after procedures, observing and managing therapeutic or possible complications resulting from the procedure. Knowledgeably initiates and performs nursing measures to prevent complications by recognizing deviations and/or changes in patient's conditions. Recognizes and responds/initiates emergency measures. Responds promptly to emergency situations in the clinic including but not limited to administering CPR. Demonstrates and applies the use of VA computer system efficiently and safety, adhering to facility and VA Policies. Active participant in PACT delivery system to include team huddles, pre-planning and team meetings. Maintains positive and constructive professional working relationships with patients, families, medical staff, volunteers, and co-workers. Observing, recording, and reporting changes in the patient's condition. Assisting in the development and revision of the patient-driven holistic life care plan. Demonstrating active collaboration, communication and problem solving with Core Team (Patient, Primary Care Provider, RN Care Coordinator and Clerk) and Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient). Maintaining confidentiality of patient / employee information electronically, verbally, and written. Working and functioning within a safe manner including infection control and operation of equipment. Utilizing nursing research, evidence-based practice, and literature in daily practice. Possessing an understanding of the changes associated with aging and principles of growth and development relevant to the adult and geriatric population. Assessing and interpreting data relating to the patient's current health status, which will facilitate the delivery of care appropriate to the age specific needs of the adult and geriatric patient. Additional clinic duties as assigned. Work Schedule: Full-Time, Monday through Friday 8:00 am to 4:30 pm. Schedule may be adjusted to support facility needs. Virtual: This is not a virtual position.
