Social Worker - Quality Management

POSITION SUMMARY: The Quality Management Social Worker is responsible for assisting Medicare patients enrolled in the Case Management, Value Based Care programs and their families in dealing with personal, social, emotional, financial and environmental problems which result from their illness or treatment.
