This job will close when we have received 100 applications, which may be sooner than the closing date.</p><h3>Major Duties</h3><p>RN Nurse Navigator for the Oncology Clinic. The Oncology Nurse Navigator (ONN) serves as a key member of the oncology care team, providing clinical expertise, education, and coordination of care for patients throughout their cancer journey. This role focuses on guiding patients and families through complex treatment plans, ensuring timely access to services, and promoting continuity of care across multiple disciplines. Navigators are critical members of interprofessional care teams that identify and eliminate barriers to timely access to cancer care. An Oncology Nurse Navigator has oncology-specific clinical knowledge to assist patients with overcoming healthcare system barriers. Key Responsibilities Patient Navigation & Care Coordination Navigation services to facilitate care transitions through diagnosis, treatment, survivorship, and end-of-life care. Serve as the primary point of contact for oncology patients and families throughout the cancer journey. Coordinate diagnostic, treatment, and follow-up appointments across various specialties, both within the VA and in the community. Facilitate communication between patients, families, Oncology Providers (MDs & NP), Radiation Oncologists, Radiologists, Pathologists, Surgeons, nursing staff, pharmacists, dieticians, social services, community care and various other departments. Reduce delays in cancer care through active coordination of care and follow-up by identifying and eliminating barriers, or delays in care. Medication administration as needed or assigned. Collaboration with Nurse Manager, or designee to ensure program success by communicating needs or concerns in a timely manner. Clinical Assessment & Education Perform comprehensive nursing assessments and monitor patient progress. Educate patients and families on diagnosis, treatment options, side effects, and self-care strategies. Provide emotional support and connect patients to psychosocial resources. Apply the nursing process to provide education and resources that facilitate informed decision-making and timely access to quality health and psychosocial care. Multidisciplinary Collaboration Participate in weekly Tumor Board Conferences, Oncology Staff Meetings, and other meetings as assigned. Collaborate with providers and the multidisciplinary team to ensure individualized care plans. Quality & Compliance Document all patient interactions and care plans in the electronic health record (EHR). Ensure compliance with clinical protocols, regulatory standards, and best practices. Contribute to quality improvement initiatives and patient satisfaction programs. Qualifications Education: Bachelor of Science in Nursing (BSN) or Masters of Science in Nursing (MSN) preferred. Certification: ONS Chemotherapy /Biotherapy Provider Certification and Oncology Certified Nurse (OCN) preferred. Experience: Preferred 3-5 years full time nursing experience as an Oncology or infusion nurse who has administered chemotherapy & immunotherapy or performed case management/navigation in Oncology. Consult or referral management skills preferred. Skills: Strong communication, critical thinking, and organizational skills. Ability to work independently and as part of a team. EDRP - This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
