Registered Nurse (Womens' Health Care Coordinator)

<p>The Registered Nurse (RN) Women's Health Care Coordinator (WHCC) is responsible for providing competent, evidence-based care to assigned patients. The RN assesses, plans, implements, and evaluates care based on age-specific components, assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>Start your VA career here Aurora Chamber of Commerce The Women's Health Care Coordinator (WHCC) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Duties of the position are, but not limited to: Influence care outcomes by collaboration with members of the interdisciplinary team. Knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Provide patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing. Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinate care with multiple providers across all levels and sites of care. Assure services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to Veterans. Provide oversight of the patient's journey through the diagnostic and treatment continuum, ensure that all steps in the process are scheduled, timely and completed, while tracking the plan of care. Contact Veterans, facilitate appointments and authorizations, track receipt of test results and other medical documentation, and notify providers of needed follow up care. Interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Anticipate and implement preventive patient care measures. Provide guidance and education to patients and caregivers and collaborate with other professionals at all levels. Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Ensure the plan of care and educational requirements are individualized and tailored specifically to support each individual patient's needs. Report status of service needs and contribute to problem resolution and modification of processes as necessary. Evaluate care and outcomes to ensure timely and appropriate provision of sex-specific services. Advocate for holistic care through patient/family education and provider interaction and implement an educational plan to meet the patient care needs. Deliver and improve WH care through collaborative strategies with others. Evaluate practice in an ongoing process, based on best evidence. Provide peers with informal constructive feedback for improvement. Foster a safe and supportive environment conducive to the professional development of healthcare professionals. Contribute professional nursing perspective in discussions with the interdisciplinary team. Partner with others to effect change and produce optimal outcomes. Support colleagues through knowledge sharing to provide safe, quality nursing care. Share educational findings, experiences, and ideas with peers. Participate in activities and strategies to sustain evidence-based-practice. Perform other duties as assigned.
