ABOUT THIS ROLE
<p>The Women's Health Care Coordinator is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. Assesses, plans, implements, and evaluates delivery of care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>The Registered Nurse -Women's Whole Health Care Coordinator assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. The duties of the position include but are not limited to: Assists women Veterans to efficiently obtain sex-specific services and care Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing maternity care, fertility care, gynecologic services, mammography, cervical cancer screening, and/or bone density testing Coordinates care with multiple providers across all levels and sites of care. Provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the plan of care. Coordinates and facilitates, as needed, WH care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. Ensures outside referral agencies and their business and clinical practices follow regulatory standards. Provides guidance and education to patients and caregivers and collaborates with other professionals at all levels. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Ensures the plan of care and educational requirements are individualized and tailored specifically to support each individual patient's needs. Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement.
