ABOUT THIS ROLE
<p>The Ralph H. Johnson VA Health Care System in Charleston, SC is seeking a Registered Nurse (Clinical Assessor) to join the Caregiver Support Program (CSP). The incumbent conducts caregiver and Veteran assessments, coordinates care, provides education and support, collaborates with interdisciplinary teams, and connects Veterans and caregivers with VA and community resources to promote quality, Veteran-centered care</p><h3>Major Duties</h3><p>Duties: Conduct comprehensive assessments of Veterans and caregivers to determine eligibility and ongoing participation in the Caregiver Support Program (CSP). Complete home visits, face-to-face, telehealth, and telephone assessments in accordance with program requirements. Assess Veterans' physical, cognitive, emotional, and functional status, as well as caregiver needs and burden. Develop, implement, and evaluate individualized plans of care using the nursing process. Provide education, training, counseling, and support to Veterans, caregivers, families, and staff regarding caregiver resources and services. Coordinate care and collaborate with interdisciplinary teams to ensure comprehensive, Veteran-centered care. Facilitate referrals to VA programs, community agencies, and other healthcare resources to meet Veteran and caregiver needs. Monitor caregiver participation and Veteran outcomes while ensuring compliance with Caregiver Support Program requirements. Document assessments, interventions, and care coordination activities in the electronic health record. Advocate for Veterans and caregivers to promote access to appropriate clinical and supportive services. Participate in program evaluation, performance improvement, and quality management activities. Maintain knowledge of VA policies, directives, and caregiver support program requirements while ensuring compliance with regulatory standards. Serve as a clinical resource and collaborate with internal and external stakeholders to support the mission of the Caregiver Support Program. Perform other duties as assigned in support of the Caregiver Support Program and organizational goals. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical evaluation. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
