ABOUT THIS ROLE
<p>The RN in this position functions as the Medical Center's Grant and Per Diem Liaison/Case Manager for VAMC's Homeless Veterans Grant and Per Diem programs and Extended Case Management program. Responsible for monitoring and coordination of services provided by these GPD community-based programs funded under VA's Homeless Providers Grant and Per Diem Program. This is an essential and critical part of VHA, vital for providing safe transitional housing and supportive services for homeless Veterans.</p><h3>Major Duties</h3><p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. VA medical centers must ensure that Veterans in community-based funded programs are receiving quality services. The liaison/case manager is responsible for the oversight of these services as well as clinical case management for Veterans receiving these services. Driving and transporting Veterans in a government vehicle is a major part of the job duties. Having a valid driver's license and being able to pass the VA Driver's Physical is contingent on performing the duties of this position. Duties and responsibilities included but are not limited to: Executes position responsibilities that demonstrate beginning leadership skills and participates in specific program experiences Demonstrate creative approaches to the management of program aspects of complex patient care Demonstrates performance and beginning leadership skills that start to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care Responsible for leading a facility wide program for all services within the organization that meets the requirements of National Directives Evaluates the program, collecting and submitting facility performance measures Provides case management at GPD funded sites as described in the GPD program rules, regulations, and directives Utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities Coordinates care including serving as an advocate on behalf of Veterans to ensure comprehensive service delivery and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies Monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application Facilitate compliance or corrective actions and, when appropriate, develops, facilitates, and monitors plans of correction Member of the HCHV coverage team providing occasional coverage, completing intakes, consults, referrals Providing customer service to walk-ins and monitoring the HCHV coverage phone on days with assigned coverage Completes consults and monitors the homeless team's pager as an active part of the coverage team Share coverage duties with the HCHV Coordinator for Deshon Place (half GPD/half HUD-VASH located on Butler VAMC property) Provides services and oversight of the GPD funded programs including verification of Veteran status and eligibility for participation in the GPD Program, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, and collecting and submitting participant information Coordinates and participates in program site inspections in accordance with the GPD Program rules, regulations, and directives Records inspection deficiencies and establishes methods to monitor corrective actions Engages in coordination with community agencies, organizations, and groups Participates in VISN and GPD conference calls and attends training offered by the GPD National Program Office Participates in program planning and development, advising the supervisor of shifting trends with caseloads Performs accountability functions: document all contacts with patients/families in the medical charts completes statistical, administrative, staffing studies and all other reports in accordance with VA Medical Center Documents in the patient's medical record in accordance with medical center policies and requirements Progress notes are timely, reflecting progress toward treatment goals Outcomes are evaluated and documented Performs other related duties as assigned Pay: Competitive salary, regular salary increases, potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: 8:00am - 4:30pm Monday - Friday Telework: Not Available Virtual: This is not a virtual position.
