<p>The Fargo VA Health Care System is recruiting for 1 full-time Coordinator for the Tribal Housing and Urban Development - Veterans Affairs Supportive Housing (HUD VASH) program. This vacancy will be filled with either a Social Worker (Program Coordinator), Licensed Professional Mental Health Counselor (Program Coordinator), or a Registered Nurse Coordinator. Registered Nurses should apply under this announcement.</p><h3>Major Duties</h3><p>The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary, regular salary increases, potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Duties of the Registered Nurse - Tribal HUD VASH Urban Coordinator include, but are not limited to: Serve as a champion and leader for the Tribal HUD-VASH Program, providing ongoing coordination and implementation of program activities in collaboration with Integrated Care Communities and systemwide care teams. Provide clinical case management services to Veterans with complex medical, psychosocial, and behavioral health needs, delivering care in person, by phone, V-Tel, or through home telehealth. Develop, implement, and monitor individualized plans of care, ensuring high-risk Veterans receive appropriate follow-up, intensive monitoring, and seamless transitions between levels of care, including primary care, HBPC, and palliative services. Coordinate interdisciplinary care, collaborating with providers across multiple sites of care to support Veteran housing stability and community reintegration, using recovery-oriented and Housing First principles. Conduct assessments to determine Veterans' ability to live independently, contributing to eligibility determinations and developing individualized Housing Stability Plans. Evaluate patient care outcomes and program processes, identifying measures that demonstrate program success and using findings to support education, accountability, and continuous improvement. Demonstrate leadership in evidence-based nursing practice, sharing knowledge with peers, providing constructive feedback, and contributing to a supportive professional environment. Perform program development and operational duties, maintaining compliance with program requirements, participating in quality improvement initiatives, and traveling frequently (up to 75%), including transporting Veterans when necessary, in support of program goals. Work Schedule: Monday-Friday, 7:30 a.m. - 4:00 p.m. or 8:00 a.m. - 4:30 p.m. Telework: Ad hoc telework available. (Employees who Telework on an occasional, episodic, or short-term, and have completed required telework training and telework agreement) Virtual: This is not a virtual position. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
