<p>The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse assigned within Ambulatory Care. The Traveling Veteran Coordinator (TVC) Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care, and is assigned to Veterans requiring complex care coordination or specialized case management services.</p><h3>Major Duties</h3><p>Major Duties Include, but are not limited to the following: Responsible and accountable for all elements of the nursing process when providing and/or supervising patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care for traveling Veterans either vested in another Veterans Affairs (VA) facility and traveling to the local catchment area, or traveling outside of the local catchment area, focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills. Serves as the primary point of contact these Veterans and their families or caregivers and communicates with the health care team while leading complex care coordination and case management efforts. Duties are multifaceted and may be divided between RN functions of the TVC Program which includes consult management, care management and coordination of benefits. Serves as a liaison for the Veteran between VA facilities and is responsible for managing the care of the Veteran throughout the process. Works collaboratively as a member of the interdisciplinary team, makes caring for the Veteran their priority while in the work setting, and demonstrates customer service principles in all aspects of work. Participates in activities and strategies to sustain an evidence-based-practice culture. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
