<p>The Traveling Veteran Coordinator Registered Nurse position is aligned with Primary Care service line at the Chillicothe VA Medical Center, Chillicothe, Ohio. Traveling Veteran Coordinator Registered Nurse is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.</p><h3>Major Duties</h3><p>Duties and responsibilities include but are not limited to: Assists with facilitation of physician/clinical consultations, as needed, to render timely clinical decisions to expedite care. Serves as the point of contact for transfers out to higher levels of care and coordinates transportation as needed. Responsible for tracking data related to transfers out and admissions to acute mental health. Assumes responsibility for the coordination of care for traveling Veterans either vested in another Veterans Affairs (VA) facility and traveling to the local catchment area, or traveling outside of the local catchment area, focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Consistently communicates and treats customers (patients, visitors, volunteers, and all Medical Center staff) in a courteous, tactful, and respectful manner. Provides the customers with consistent information according to established policies and procedures. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents). Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). You may be required to serve a probationary period.
