ABOUT THIS ROLE
<p>The Registered Nurse Transfer coordinator is directly responsible for the facilitation and monitoring of the community hospital referrals to the VA facility, VA to VA, and VA to community hospital. This individual is substantially responsible for collaborating with other staff for appropriate oversight and management of emergency care received through community hospitals.</p><h3>Major Duties</h3><p>Duties and responsibilities, but not limited to: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. This includes working with providers to coordinate clinical and administrative information needed to make prudent decisions regarding services, and providing accurate, timely and comprehensive analysis and reports to VISN 9 leadership and VA leadership. In addition, this position has oversight of the emergency reporting system for emergent admissions to community hospitals utilized by VHA, VISN, and local VA. This position will follow Code of Federal Regulations (CFR) when reviewing emergency episodes of care for 1703, 1725, and 1728. This position collaborates with Care in the Community, House Supervisor, Traveling Veteran Coordinator, and Bed Control. The Transfer Nurse actively participates in activities and utilizes clinical knowledge and skills to insure appropriate level of care, utilizes standardized criteria to conduct clinical reviews for appropriateness of admissions, continued stay, utilization of outpatient and ancillary services, collects data, participates in analysis and reporting, and participates in IDT teams, and serves as a liaison with the clinical staff to facilitate quality care that is cost effective. The nurse will work closely with Care in the Community to provide care coordination and transition back to VA for Veterans who have utilized community hospitals for emergency care.
