<p>This Registered Nurse position is located in the Medical Service at the West Roxbury campus location of VHA Boston. This position is full-time at 40 hours per week.</p><h3>Major Duties</h3><p>Collaborate with inpatient teams to identify Veterans with complex medical and social needs and serious illness for enrolment in Supportive Coordinated Transitional Care (CTraC) Program Provide post-discharge care coordination, education, anticipatory symptom management, and alignment with goals of care Coordinate care between inpatient and outpatient palliative care, geriatrics, prosthetics, rehabilitation medicine, and pharmacy teams Provide outpatient clinic support to include triage of consults, hospice referrals, responding to veteran and family calls and secure messages, symptom assessment and collaboration with providers. Facilitate hospice referrals to home and other facilities including in other states. Collaborate closely with hospice agencies and social work. Provide nursing and clinician education on hospice, palliative care and Supportive CTraC at VA, affiliates, and MA Hospice Federation. Participate in national CTraC community and mentor Supportive CTraC nurses across the VA. Collect data on Supportive CTraC program to ensure value added, improved care, and cost avoidance. Continually evaluate program effectiveness in collaboration with GEC leads. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Notifications: This position is in the Excepted Service Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. This position is covered by a special rate.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
